The purpose of this essay is to investigate and critically analyse some of the formative factors which led to the spiritual maturation of a leading Vīraśaiva saint, Basava. This inquiry focuses on a single event in the life of this great reformer of medieval times, i.e. his spiritual conflict leading to his rejection of the upanayana ceremony. The study will proceed through an investigation of the earliest and subsequent sources which veil the personality of Basava. The traditional view will be (...) challenged as one-sided. The civil war which ended in the victory of Octavian and the suicide of Antony and Cleopatra is one of the most exciting but most obscure periods of Roman history, obscure mainly because the victor succeeded in imposing bis version of affairs upon his countrymen and through them on posterity. That is not to say that his version is necessarily completely false: the danger that threatened Rome was a real one, the national feeling that resulted in the coniuratio totius Italiae (...) of 32 B.C. and that inspired Virgil and Horace later was not an artificial growth, though it was carefully tended. But in kindling the requisite war-feeling and in rousing the necessary enthusiasm both sides had to propagand for themselves, and in ancient times propaganda often became a matter of personal abuse and mud-slinging. In this Octavian's agents were perhaps more successful, though few nowadays would accept the conventional portraits of Antony and Cleopatra as anywhere near the truth. But Antony's propaganda, though not so effective, was not obliterated by Octavian's victory; indeed a great deal of it is still preserved and masquerades as fact in histories of the period, where Octavian's personal character suffers badly. In this paper I hope to show that a conception of human rights requires a view of the social ideal and the good life, and requires a view of the nature of human community. But what I say in favor of these points hardly amounts to a demonstration. Instead I try to exhibit how we think and talk about rights in general, and what the presuppositions of such thought and talk are. Throughout, I emphasize the pragmatic side of rights-discourse and (...) I try to establish some of the contexts in which such discourse can be significantly employed. Much of what I say is motivated by the consideration that an historically important factor in the generation of various rights, their acknowledgement and implementation, and their extension to increasingly larger segments of the population, has been the claims put forward and the demands made by individuals and groups against individuals and groups. This factor constitutes a suggestive clue for the development of a theory of rights. The treatment here, however, is largely programmatic and fragmentary. The space allotted does not permit more. Moreover, I lead up to—but do not go into—the social ideal and the human community. The title of this essay indicates its limited scope. The above points are, of course, hardly novel; many older writers laid great stress upon them. But of late they have not received the attention that they in my view deserve. What does it mean for someone to maintain a conception of human rights? How is this conception given any concrete content? The answers to these questions, I believe, ultimately lead to a consideration of social ideals and the human community. What follows is meant to stimulate new thinking on these old-fashioned topics. Women who had had breast cancer and had been enrolled in a large genetic breast cancer epidemiological study were interviewed about their experience of participation in the study, their attitudes to the confidentiality of data, and the feedback of personal and general research results. Collection of family history information seemed more salient in indicating the genetic nature of the study than the enrolment information sheet. There were no concerns about confidentiality.While participants would have welcomed general feedback about the results (...) of the study and were critical that this had not been provided, the feedback of personal information proved complicated and, sometimes, difficult. It is suggested that individual feedback of genetic test information in epidemiological studies should be undertaken only when there are specific reasons. (shrink)

It is now time for bioethics to take full account of the problems of health disparities and structural injustice that are made newly urgent by the COVID-19 pandemic and the effects of climate change. Churchill make the case for a more social understanding and application of justice, a deeper humility in assessing expertise in bioethics consulting, a broader and more relevant research agenda, (...) and greater appreciation of the profound health implications of global warming. One hundred years separates the birth of T. Campanella from that of G. B. Vico. This collection of essays, published in 1969 by the Archives of Philosophy, is the end result of a congress held in Rome commemorating the birth of both philosophers. Campanella and Vico are celebrated in the same volume on the assumption of ideal affinities besides the accidental circumstances of having been born a century apart. Unfortunately, only one essay is expressly devoted to the analysis of Campanella's (...) and Vico's metaphysics. While the first two papers consider Campanella's medical biography and his epistemology, the last four are dedicated solely to various aspects of Vico's thought. On Iacobelli Isoldi falls the responsibility of justifying the title of the volume. But her attempt to demonstrate a family resemblance between Vico's and Campanella's metaphysics remains farfetched. For Campanella's way of solving the theoretical problem arising from Descartes' dualism issues in inevitable skepticism. Campanella never abandoned the position that the cogito is a vehicle of certainty, but not a criterion of truth. Vico on the other hand, overcomes his initial skepticism by transforming the criterion of truth into a criterion of certainty. In the New Science he demonstrates how man can possess true knowledge of the social and historical sciences. To demonstrate that this symposium had been assembled for reasons other than physical convenience, more papers on possible relations between the Dominican friar of Cosenza and the professor of eloquence of Naples would have been a desideratum.--L. This volume is a collection of testimonia and fragments of the Presocratics designed to introduce the Italian high school student to an understanding of what philosophy is. This purpose is so praiseworthy that it should deter the specialist from raising his eyebrows at the volume's shortcomings. Curi has selected only fragments of Thales, Anaximander, Anaximenes, Pythagoras, Xenophanes, and the Eleatics. Empedocles, Anaxagoras and the Atomists are excluded from the selections, presumably because the editor holds, with many Italian scholars, the questionable (...) theory that by "Presocratics" is understood only those thinkers who lived before the birth of Socrates. The translation of texts from the sixth edition of Diels and Kranz is accurate and readable, and the selections of testimonia are more comprehensive than the ones found in the textbooks of Burnet, Nahm, and Robinson. It is pedagogically interesting to ask what the very young may learn from this volume. The answer lies in its short introduction and in the critical footnotes attached to each fragment. Unlike the aforementioned English and American authors, Curi avoids textual exegesis and analysis of the mythopoeic background of Homer and Hesiod to which the early philosophic views may be related. He centers his own comments on the intricate issues of ancient and modern historiography. He analyzes the history of ancient historiography from Aristotle to Mondolfo and concludes that Wilamowitz, Stenzel, and Jaeger are the only historiographers who have been able to capture the complexity of Presocratic philosophy. Curi's method of teaching philosophy and his own philosophical views goes beyond the scope of this notice, however we can not help but remark how much Croce's spirit still pervades the Italian class-room.--L. This book was edited by J. Dopp from a nearly completed manuscript of Feys, and Dopp has added a bibliography and interstitial material where required. Feys has tried to survey the whole range of modal logic with the exception of topological and algebraic interpretations of modal structures and operators. First we are introduced to the modal propositional calculi through both historical and non-modal viewpoints; there are then formulated five systems of modal logic, with variants, with the standard required reduction and (...) other properties, and a number of useful metatheorems about decision-methods and derivability are proved. The chapter on modal functional logic first handles a modal system with first-order quantification, then extends it to include class-abstraction. Among other topics appearing in the book are extensions of modal calculi and the seemingly-paradoxical one of identity in modal functional calculi. Although there are some rough spots caused by the state of the manuscript itself, this book is more than adequate for extensive study in modal logic and should serve that purpose well.—P. J. M. The preoccupation evinced by Italian scholars for Protagoras and the Sophistic movement is extraordinary and somewhat comparable to the interest which American philosophers have recently shown in Parmenides and the Eleatics. Since the Protagoras revival in the 'Forties' numerous Italian editions of the Sophistic fragments have been published and a constellation of contradictory reconstructions of Protagoras' thought has been offered to a perhaps unduly tolerant public. In the present study Zeppi intends to refute all extant historiography from Gomperz to Capizzi (...) in the name of a unitary interpretation based upon the internal consistency of Protagoras' ethical and epistemological doctrines. Such an interpretation will do justice, Zeppi believes, to the complexity of Protagoras' thinking while revealing the central position he occupies in the philosophical tradition before Plato. Accordingly Zeppi devotes the first part of his essay to Protagoras' doctrines and the second to earlier Presocratics and the Sophists in general. Finally he offers thought-provoking studies on the Anonymus Iamblichi, on the Cyrenaic school, on Democritus' ethics and on its bearing on Protagoras. On the whole Zeppi is more convincing in demolishing the views of his predecessors than in presenting his own ideas. Nevertheless his hypothesis of Protagoras as an ethical emotivist, a sense-datum theorist and a thinker divorcing truth from morality is refreshingly questionable.--L. M. P. (shrink)

The author has succeeded in producing a volume which will be valuable to anyone who wishes to acquire a more catholic view of British analytic philosophy than the one offered in the works of Urmson, Warnock, and Ayer. The bibliography contains 698 items, and each of the ten chapters is amply footnoted with suggestions for further investigations. In the first chapter, Riverso meets the charge of antihistoricism by arguing convincingly that twentieth century analysis is the culmination of a movement (...) which began with Descartes' Regulae ad directionem ingenii. In the following three chapters he considers the entire Cambridge movement from its origin in B. Russell's and G. E. Moore's refutation of idealism to the disintegration of logical atomism in the second Wittgenstein. Italian readers will find especially useful Riverso's clear discussion of the impact of Wittgenstein's Tractatus on the Vienna circle, of his analysis of Ayer's radical refutation of metaphysics, as well as of the polemics of Lazerowitz and Malcolm against the principle of verifiability. In the final four chapters, Riverso takes on the arduous task of telling the story of the whole Oxonian movement. He clarifies beautifully the reasons for the transformation from normal analysis to ordinary language, and finally to the various types of grammatical elucidations of Ryle, Austin, and Strawson. Riverso dedicates a chapter each to the language of perception, the concept of person, religious language, and metaethics. Of Peter Abelard's logical writings, the fruit of thirty-five years of his activity as a teacher and dialectician, there have been to date only four editions worthy of note: that of Cousin in 1836, of Geyer in 1919 and 1936, of Dal Pra in 1954, and lastly the critical reconstruction of Abelard's literal glosses by L. M. Rijk in 1956. With this handsome second edition of minor but relevant glosses of Abelard, Dal Pra aims to fill the gap still existing (...) in the previous collections. His volume includes Abelard's writings found in the Paris BN manuscripts n. 13.368 and 7493. Chronologically the former belongs to the period when Abelard was given the chair of dialectic at the University of Paris in 1114 and the latter to Abelard's more mature period when he began original speculations on the logical status of propositions in 1121. Dal Pra argues at great length that the glosses in the first manuscript are to be read as the best and only introduction to Abelard's own work on dialectic. Hence he suggests against Rijk the title Introductiones Dialecticae rather than Introductiones Parvulorum. He assembles them according to what he believes was Abelard's intellectual development at the time. They are: the glosses to Porphyrius, to Aristotle's Categories, to the De Interpretatione, and to Boethius' De Devisione. [[sic]] The second section of the volume contains the glosses Super Topica where Abelard discusses among other subjects his first position on the problem of universals. Against Geyer who had consigned these works to the literal glosses of Abelard, Dal Pra offers strong evidence for locating them in the bulk of Abelard's De Ingredientibus precisely between the De Interpretatione and the De Syllogismo Hypothetico. The modernity of Abelard's logical theory is well known among contemporary logicians. Dal Pra's finely planned and handsomely executed edition makes an excellent contribution to the history of medieval logic as well as to contemporary logical theories on names and propositions.--L. M. Background Regarding controversial medical services, many have argued that if physicians cannot in good conscience provide a legal medical intervention for which a patient is a candidate, they should refer the requesting patient to an accommodating provider. This study examines what US physicians think a doctor is obligated to do when the doctor thinks it would be immoral to provide a referral. Method The authors conducted a cross-sectional survey of a random sample of 2000 US physicians from all specialties. The (...) primary criterion variable was agreement that physicians have a professional obligation to refer patients for all legal medical services for which the patients are candidates, even if the physician believes that such a referral is immoral. Results Of 1895 eligible physicians, 1032 (55%) responded. 57% of physicians agreed that doctors must refer patients regardless of whether or not the doctor believes the referral itself is immoral. Holding this opinion was independently associated with being more theologically pluralistic, describing oneself as sociopolitically liberal, and indicating that respect for patient autonomy is the most important bioethical principle in one's practice (multivariable ORs, 1.6–2.4). Conclusions Physicians are divided about a professional obligation to refer when the physician believes that referral itself is immoral. These data suggest there is no uncontroversial way to resolve conflicts posed when patients request interventions that their physicians cannot in good conscience provide. (shrink)

Recent evaluation of the practice of euthanasia and related medical decisions at the end of life in the Netherlands has shown a slight decrease in the frequency of physician-assisted death since the enactment of the Euthanasia Law in 2002. This paper focuses on the absence of euthanasia cases concerning patients with dementia and a written advance euthanasia directive, despite the fact that the only real innovation of the Euthanasia Law consisted precisely in allowing physicians to act upon such directives. The (...) author discusses two principal reasons for this absence. One relates to the uncertainty about whether patients with advanced dementia truly experience the suffering they formerly feared. There is reason to assume that they don’t, as a consequence of psychological adaptation and progressive unawareness (anosognosia). The second, more fundamental reason touches upon the ethical relevance of shared understanding and reciprocity. The author argues that, next to autonomy and mercifulness, “reciprocity” is a condition sine qua non for euthanasia. The absence thereof in advanced dementia renders euthanasia morally inconceivable, even if there are signs of suffering and notwithstanding the presence of an advance euthanasia directive. This does not mean, however, that advance euthanasia directives of patients with dementia are worthless. They might very well have a role in the earlier stages of certain subtypes of the disease. To illustrate this point the author presents a case in which the advance directive helped to create a window of opportunity for reciprocity and shared decision-making. (shrink)

Despite differences in their function and domain-specific elements, syntactic processing in music and language is believed to share cognitive resources. This study aims to investigate whether the simultaneous processing of language and music share the use of a common syntactic processor or more general attentional resources. To investigate this matter we tested musicians and non-musicians using visually presented sentences and aurally presented melodies containing syntactic local and long-distance dependencies. Accuracy rates and reaction times of participants’ responses were collected. In (...) both sentences and melodies, unexpected syntactic anomalies were introduced. This is the first study to address the processing of local and long-distance dependencies in language and music combined while reducing the effect of sensory memory. Participants were instructed to focus on language (language session), music (music session), or both (dual session). In the language session, musicians and non-musicians performed comparably in terms of accuracy rates and reaction times. As expected, groups’ differences appeared in the music session, with musicians being more accurate in their responses than non-musicians and only the latter showing an interaction between the accuracy rates for music and language syntax. In the dual session musicians were overall more accurate than non-musicians. However, both groups showed comparable behavior, by displaying an interaction between the accuracy rates for language and music syntax responses. In our study, accuracy rates seem to better capture the interaction between language and music syntax; and this interaction seems to indicate the use of distinct, however, interacting mechanisms as part of decision making strategy. This interaction seems to be subject of an increase of attentional load and domain proficiency. Our study contributes to the long-lasting debate about the commonalities between language and music by providing evidence for their interaction at a more domain-general level. (shrink)

This work is a defense of positivism in social theory: Addis is committed to the view that the substantive concerns of physical and social science are essentially the same. Indeed, he states that an adequate philosophy of society presupposes an adequate philosophy of science. He therefore begins with an analysis of various themes from the latter: causation, the notions of process and closed systems, and determinism. It is in fact a basic presumption of the book that some refined theory of (...) determinism is true. The author argues that all "social objects," such as those posited by Durkheim, are no more than properties of, or relations between, individual social beings. In the course of this argument, he develops a detailed treatment of reduction of the social into the physical sciences. It is his belief that logical analysis of the notion of reduction is essential to an understanding of social theory. Reduction is examined in two phases. First, he elaborates on his contention that social objects reduce to individual human beings. Second, he examines the reduction of socio-historical explanations to psycho-physical laws. He then proceeds to apply the preceding discussions to problems of philosophy of history, the relation of ideas to historical process, freedom and purpose in history, and a critique of "Marxism.". Coastal ecosystems are increasingly dominated by humans. Consequently, the human dimensions of sustainability science have become an integral part of emerging coastal governance and management practices. But if we are to avoid the harsh lessons of land management, coastal decision makers must recognize that humans are one of the more coastally dependent species in the biosphere. Management responses must therefore confront both the temporal urgency and the very real compromises and sacrifices that will be necessary to achieve a sustainable (...) coastal ecosystem, one that is economically feasible, socially just, and ecologically sound. Study objectives The emergency department (ED) provides an arena for patient enrolment into a variety of research studies even for non-critically ill patients. Given the types of illness, time constraints and sense of urgency that exists in the ED environment, concern exists about whether research subjects in the ED can provide full consent for participation. We sought to identify enrolled research subjects' perspectives on the informed consent process for research conducted in the ED. Methods This was a prospective, observational study (...) of ED subjects, 18 years or older, who had been approached to participate in research in the ED and who were judged to have decision-making capacity. Exclusions were critical illness and refusal to participate. Subjective were followed up within 1 week after enrolling using structured phone interviews by trained interviewers. Results During the study period, 229 eligible patients were approached to participate in both a target study and this study. Of these, 66% (150/229) agreed to participate in this study, at least to the extent of allowing us access to their demographic data. The study participant group was similar in terms of gender to this particular ED's patient population but had significantly more African-Americans and persons older than 45. Conclusion Despite rigorous time constraints and rapid throughput times, the majority of subjects who consented to research participation in the ED felt that they were sufficiently informed and had adequate time to decide to participate. Ion channels are transport proteins present in the lipid bilayers of biological membranes. They are involved in many physiological processes, such as the generation of nerve impulses, hormonal secretion, and heartbeat. Conformational changes in the ion channel-forming protein allow the opening or closing of pores to control the ionic flux through the cell membranes. The opening and closing of the ion channel have been classically treated as a random kinetic process, known as a Markov process. Here the time the channel (...) remains in a given state is assumed to be independent of the condition it had in the previous state. More recently, however, several studies have shown that this process is not random but a deterministic one, where both the open and closed dwell-times and the ionic current flowing through the channel are history-dependent. This property is called long memory or long-range correlation. However, there is still much controversy regarding how this memory originates, which region of the channel is responsible for this property, and which models could best reproduce the memory effect. In this article, we provide a review of what is, where it is, its possible origin, and the mathematical methods used to analyze the long-term memory present in the kinetic process of ion channels. Medical teams care for severely premature infants under conditions of emergency and uncertainty that make parental involvement very difficult. Parents can be invited into a decisional relationship with the team that enables them to assess more fully the meaning of their child's illness.

The strength of this book is not a novelty of analysis, but rather its clarity and readability. The author intends it to be a useful teaching tool, "a genuine introduction, presupposing neither background in philosophy nor familiarity with the issues...." It is well arranged and quite successful as an introductory commentary. The book would appear to work best as a secondary source, supplementing more extensive philosophical texts. It need not be limited to courses specifically dealing with social and political (...) philosophy. The first chapter offers an elementary explanation of the nature of philosophical discourse in general; chapters three, "Freedom," and five "Utilitarianism and Justice," would be useful in a course on ethics. The inconsistent definition of empathy has had a negative impact on both research and practice. The aim of this article is to review and critically appraise a range of definitions of empathy and, through considered analysis, to develop a new conceptualisation. From the examination of 43 discrete definitions, 8 themes relating to the nature of empathy emerged: “distinguishing empathy from other concepts”; “cognitive or affective?”; “congruent or incongruent?”; “subject to other stimuli?”; “self/other distinction or merging?”; “trait or state influences?”; “has (...) a behavioural outcome?”; and “automatic or controlled?” The relevance and validity of each theme is assessed and a new conceptualisation of empathy is offered. The benefits of employing a more consistent and complete definition of empathy are discussed. (shrink)

Rationale, aims and objectives Given the increasing prevalence of cataract and demand for cataract extraction surgery, patients must often wait to undergo this procedure. We validated a previously developed priority scoring system in terms of clinical variables, pre-intervention health status, appropriateness of surgery and gain in visual acuity (VA) and health-related quality of life (HRQoL).Methods Explicit prioritization criteria for cataract extraction created by a variation of the Research and Development (RAND) and University of California Los Angeles appropriateness methodology were retrospectively (...) applied to a prospective cohort of 5257 patients on waiting lists to undergo cataract by phacoemulsification at 17 hospitals in Spain. Demographic data, clinical data and data related to surgical technique were collected by trained ophthalmologists. Patients were evaluated by their ophthalmologist before the intervention and 6 weeks afterward. They also completed, by mail, the Visual Function Index (VF-14) before the intervention and 3 months afterward.Results High-priority patients experienced greater improvement in VA and HRQoL than those classified as intermediate or low-priority (P < 0.0001), even after adjusting by VA and the VF-14 score at baseline. The time to intervention was the same for high-priority patients as it was for intermediate and low-priority patients.Conclusions The priority score we developed identified patients most likely to experience the greatest improvements from cataract extraction. Use of this tool could provide a fairer and more rational way to prioritize patients for cataract extraction. (shrink)

Background: There has for some time now been recognition that there was a relationship between exceptional creative talent and mental disorder. The works of Andreasen (2008) and others in this area have been very significant. However, most of the research has been carried out in USA and Europe. Very little has come out of Africa on the subject. Aim : To survey the beliefs of different groups within an African society, concerning the possibility of a relationship between creative talent and (...) mental disorder. To assess creativity within a community of people with a formal diagnosis of mental disorder. Materials and Methods: Some of the mythology of the Yoruba was examined for content, concerning the behaviour of certain notable individuals and the existence of psychopathology based on modern-day criteria. The beliefs of members of the general public and mental health professionals concerning the existence of a relationship between creative talent and psychopathology were surveyed using a questionnaire designed for the project. A sample of patients with formal diagnoses of affective disorder or schizophrenia drawn from two units, the Lagos State University Teaching Hospital and the Federal Neuropsychiatric Hospital Yaba, were assessed for 'Creativity.' Results : Although there are notable 'eccentric' figures in local mythology, the overwhelming majority of the people surveyed do not believe there is any relationship between creativity and mental illness. They however believe that engaging in creative activities helps the mentally ill to recover from illness. The mental health professionals, who were clinical psychologists and psychiatrists, had a significant minority who believed that a relationship does exist, and they also strongly assert that creative activity has a therapeutic effect for the mentally ill. A survey of in-patients diagnosed with schizophrenia and affective disorder failed to show a significant difference in the creativity of the two populations, as measured by the originality score of the Rorschach scale. The survey of patients is inconclusive, based on small sample size (ten patients with a diagnosis of schizophrenia, ten with bipolar affective disorder.). The linkage between formal mental disorder is only recognised by a significant minority of mental health professionals. A significant proportion of the population believe that creative activity aids recovery from mental illness. More research is required into this important subject in Africa. (shrink)

This study examined health professionals’ (HPs) experience, beliefs and attitudes towards brain death (BD) and two types of donation after circulatory death (DCD)—controlled and uncontrolled DCD. Five hundred and eighty-seven HPs likely to be involved in the process of organ procurement were interviewed in 14 hospitals with transplant programs in France, Spain and the US. Three potential donation scenarios—BD, uncontrolled DCD and controlled DCD—were presented to study subjects during individual face-to-face interviews. Our study has two main findings: (1) In the (...) context of organ procurement, HPs believe that BD is a more reliable standard for determining death than circulatory death, and (2) While the vast majority of HPs consider it morally acceptable to retrieve organs from brain-dead donors, retrieving organs from DCD patients is much more controversial. We offer the following possible explanations. DCD introduces new conditions that deviate from standard medical practice, allow procurement of organs when donors’ loss of circulatory function could be reversed, and raises questions about “death” as a unified concept. Our results suggest that, for many HPs, these concerns seem related in part to the fact that a rigorous brain examination is neither clinically performed nor legally required in DCD. Their discomfort could also come from a belief that irreversible loss of circulatory function has not been adequately demonstrated. If DCD protocols are to achieve their full potential for increasing organ supply, the sources of HPs’ discomfort must be further identified and addressed. (shrink)

This article is the lead piece in a special report that presents the results of a bioethical investigation into chimeric research, which involves the insertion of human cells into nonhuman animals and nonhuman animal embryos, including into their brains. Rapid scientific developments in this field may advance knowledge and could lead to new therapies for humans. They also reveal the conceptual, ethical, and procedural limitations of existing ethics guidance for human‐nonhuman chimeric research. Led by bioethics researchers working closely with an (...) interdisciplinary work group, the investigation focused on generating conceptual clarity and identifying improvements to governance approaches, with the goal of helping scholars, funders, scientists, institutional leaders, and oversight bodies (embryonic stem cell research oversight [ESCRO] committees and institutional animal care and use committees [IACUCs]) deliver principled and trustworthy oversight of this area of science. The article, which focuses on human‐nonhuman animal chimeric research that is stem cell based, identifies key ethical issues in and offers ten recommendations regarding the ethics and oversight of this research. Turning from bioethics’ previous focus on human‐centered questions about the ethics of “humanization” and this research's potential impact on concepts like human dignity, this article emphasizes the importance of nonhuman animal welfare concerns in chimeric research and argues for less‐siloed governance and oversight and more‐comprehensive public communication. Initial responses to questionnaires used to assess participants' understanding of informed consent for malaria vaccine trials conducted in the United States and Mali were tallied. Total scores were analyzed by age, sex, literacy (if known), and location. Ninety-two percent (92%) of answers by United States participants and 85% of answers by Malian participants were correct. Questions more likely to be answered incorrectly in Mali related to risk, and to the type of vaccine. For adult participants, independent predictors of higher (...) scores were younger age and female sex in the United States, and male sex in Mali. Scores in the United States were higher than in Mali (P = 0.005). Despite this difference participants at both sites were well informed overall. Although interpretation must be qualified because questionnaires were not intended as research tools and were not standardized among sites, these results do not support concerns about systematic low understanding among research participants in developing versus developed countries. The intrusive state has long viewed women as fetal containers. TheDobbsdecision goes further, essentially causing women to vanish when fetuses are abstracted from their relationships to pregnant persons. The ways in which women are first controlled and then made invisible are clearly connected with the move from obedience to omission that has historically affected black Americans. When personal decisionmaking and participation in democracy are regarded as threats, those threatened restrict decisional freedom and political power, deepening structural injustices relating to sex, (...) race, and poverty. Fear ofDobbshas health effects on conditions unrelated to pregnancy and connects with erasures of human value that are not health-related. We reaffirm solidarity as a countering influence. Taking account of the richly relational context in which issues like abortion and political representation arise should lead to better, more meaningful policies, making so many people impossible to unsee. Background Informed consent is a requirement for all research. It is not, however, clear how much information is sufficient to make an informed decision about participation in research. Information on an online questionnaire about childhood development was provided through an unfolding electronic participant sheet in three levels of information. Methods 552 participants, who completed the web-based survey, accessed and spent time reading the participant information sheet (PIS) between July 2008 and November 2009. The information behaviour of the participants was investigated. (...) The first level contained less information than might be found on a standard PIS, the second level corresponded to a standard PIS, and the third contained more information than on a standard PIS. The actual time spent on reading the information provided in three incremental levels and the participants' evaluation of the information were calculated. Results 77% of the participants chose to access the first level of information, whereas 12% accessed the first two levels, 6% accessed all three levels of information and 23% participated without accessing information. The most accessed levels of information were those that corresponded to the average reading times. Conclusion The brief information provided in the first level was sufficient for participants to make informed decisions, while a sizeable minority of the participants chose not to access any information at all. This study adds to the debate about how much information is required to make a decision about participation in research and the results may help inform the future development of information sheets by providing data on participants' actual needs when deciding about questionnaire surveys. On September 8, 2015, the Department of Health and Human Services issued a Notice of Proposed Rule Making to revise the Federal Policy for the Protection of Human Subjects, widely known as the “Common Rule.” The NPRM proposes several changes to the current system, including a dramatic shift in the approach to secondary research using biospecimens and data. Under the current rules, it is relatively easy to use biospecimens and data for secondary research. This approach systematically facilitates secondary research with (...) biospecimens and data, maximizing the capacity for substantial public benefit. However, it has been criticized as insufficiently protective of the privacy and autonomy interests of biospecimen and data sources. Thus, the NPRM proposes a more restrictive regime, although more so for biospecimens than data. Both the status quo and the NPRM's proposal are critically flawed. (shrink)

The figure of Peter the Tomb, various aspects of his life and activity, as well as the era called by his name, have long been the subject of study by numerous scholars, comprehensive and meticulous research. It is enough to mention the works of M.Grushevsky, A.Zhukovsky, V.Klimov, A.Kolodny, V.Nichik, O.Sarapin, L.Filipovich and V.Shevchenko, in which the phenomenon of the metropolitan, his spiritual and religious dominant, is thoroughly and thoroughly explained, the influence of P. Mogila's heritage on the past and present. (...) And yet it is impossible to talk about the exhaustiveness of this topic, because such inexhaustibility is dictated by the very universe of its multidimensional activity. This is what determines the relevance of the proposed article. And this, in turn, gives rise to reflections on the need for scientific innovative understanding of religious philosophy as a constant basis of thinking of Petro Mohyla, which, in particular, was reflected in his "Notebook", which is still a little-studied phenomenon of Ukrainian spiritual culture, for the first time it saw the world only in 1995, more than 60 years after its invention by M. Grushevsky. This work, like all narratives on the spiritual and religious themes that make up it, is an integral part of the metropolitan's religious and philosophical heritage. (shrink)

In the last decade literature focused on a “less is more” approach has been primarily represented by clinical cases describing the excesses of an aggressive, redundant, non-personalized, and non-respectful medicine. Most of these articles focus on a “more is worse” approach and centre around the downstream negative consequences of medical overuse. Having identified a gap in the literature on the experience and practice of less, rather than the harms of excess, we carried out an exploratory qualitative study into (...) how a “less is more” approach works in practice. A hermeneutic phenomenological approach was adopted to allow us to examine the realm of lived experience as a valid data source and as a path from which to understand a “less is more” approach “from the bedside.” A Phenomenology of Practice was chosen as a more specific frame for this research because of its added focus on action and practical application in professional settings. Seventy stories written by physicians, patients, nurses, caregivers, and other health professionals have been received and analysed. These stories were gathered as part of a project called “Slow Stories” which aimed to collect clinical cases that have been positively resolved by adopting a “less is more” approach to patient care. After having conducted an in-depth analysis, separately and as a group, the researchers identified five key phenomenological themes; Time to relate is time to heal; Doing more does not mean doing better; Settings for a slow medicine; Slow care at the end of life; and Personalized vs. standardized treatment. Each of these themes offers insights into how a “less is more” approach can be used in practice and illustrates how a “less is more” strategy can play a significant role in positively resolving certain clinical cases. The desire to see a sign phenomenon in different ways always has a reason to interpret it in an unbiased, panoramic way, and in some places - even allowing for contradictions in its understanding by different participants in the interpretative process. For modern humanities, this disposition is quite understandable, since it follows from its very postmodern nature, thereby defining the semantic semantic fields of the leading humanities. True, it is not so wide-spread, but instead, it is evident that Ukrainian religious (...) thinkers of the Middle Ages and Baroque have interpreted the iconic phenomena more deeply. In the story of Petro Mohyla "On the Wonderful Elder Grigory Mezhyhirsky" the metropolitan Mohyla clearly expresses the need to build before the thinking gaze of the reader his own, immanently religious-philosophical conception of the Divine, which every time would present him not just as an act of manifestation, otherwise he appears before the person, while remaining alone. The contemporary study of this issue in order to integrate its leading concepts in the religious-philosophical and general cultural discourse of the postmodern era has largely determined the relevance of the proposed article. The study of the phenomenon of "leaving" by itself in changing forms of the phenomenon of the Divine is also one of the key ones in the proposed studio, because it expresses the very meaning of the Divine as a religious, and therefore a cultural phenomenon. Introduction Families of patients with dementia may need support in difficult end-of-life decision making. Such guidance may be culturally sensitive. Methods To support families in Canada, a booklet was developed to aid decision making on palliative care issues. For reasons of cost effectiveness and promising effects, we prepared for its implementation in Italy, the Netherlands and Japan. Local teams translated and adapted the booklet to local ethical, legal and medical standards where needed, retaining guidance on palliative care. Using qualitative content (...) analyses, we grouped and compared adaptations to understand culturally sensitive aspects. Results Three themes emerged: (1) relationships among patient, physician and other professionals—the authority of the physician was more explicit in adapted versions; (2) patient rights and family position—adding detail about local regulations; and (3) typology of treatments and decisions. Considerations underlying palliative care decisions were detailed (Dutch and Italian versions), and the Japanese version frequently referred to professional and legal standards, and life-prolongation was a competing goal. Text on artificial feeding or fluids and euthanasia was revised extensively. Conclusions Providing artificial feeding and fluids and discussing euthanasia may be particularly sensitive topics, and guidance on these subjects needs careful consideration of ethical aspects and possible adaptations to local standards and practice. The findings may promote cross-national debate on sensitive, core issues regarding end-of-life care in dementia. (shrink)

The considerable attention to financial interests in clinical research has focused mostly on academic medical centers, even though the majority of clinical research is conducted in community practice settings. To fill this gap, this article maps the practices and policies in 73 community hospitals and several hundred specialized facilities around the country for reviewing clinical investigators’ financial relationships with research sponsors. Community hospitals face a substantially different mix of issues than academic medical centers do because their physician researchers are usually (...) not employees, and more of their studies are later-phase, multicenter trials. Accordingly, community physicians stand to benefit personally from per capita payments, but they tend to have less direct influence over research findings when compared with their academic peers. These and other contrasts require separate guidance for community-based organizations that takes into account their particular needs, circumstances, and institutional capacities. (shrink)

An instrument to assess 'ethical sensitivity' has been developed. The instrument presents four clinical vignettes and the respondent is asked to list the ethical issues related to each vignette. The responses are classified, post hoc, into the domains of autonomy, beneficence and justice. This instrument was used in 1990 to assess the ethical sensitivity of students in all four medical classes at the University of Toronto. Ethical sensitivity, as measured by this instrument, is not related to age or grade-point average. (...) Sensitivity increases between the 1st and 2nd year and then decreases throughout the rest of undergraduate medical training, such that the 4th-year students identify fewer issues than those entering medical school. Students expressing a career choice of family medicine identify more issues than their peers. Several problems with the use of the instrument and the interpretation of the data were found. Nonetheless, these findings, if reproducible, are important and their meaning needs further discussion. (shrink)

The aim of this study was to examine developmental differences in autobiographical memory using a novel test that assesses its semantic and episodic subcomponents. Forty-two children aged 7–13 years were asked to recall semantic information and episodic events from three different time periods. For the recalls of all events, sense of remembering or sense of just knowing was measured via the Remember/Know paradigm. Age-related differences were observed for episodic autobiographical memory whereas semantic autobiographical memory was characterized by a relative developmental (...) invariance. The increase with age was also found in the number of “Remember” responses and their justification in terms of the actual contextual information retrieved—factual, spatial, and, more especially, temporal details. These findings highlight developmental differences between the episodic and semantic subcomponents of autobiographical memory and support the view that mental ‘time travel’ through subjective time, which allows one to re-experience the past through self-awareness, is the last feature of autobiographical memory to become fully operational. Child-to-parent violence has dramatically risen in the last decade, becoming a concerning issue in many countries, so research on this issue has also increased. However, most of the studies on this topic have been conducted with samples of adolescents, and very few with samples of parents. In addition, the variety of assessment instruments does not reflect the elements of this type of violence. Thus, the current study was aimed to examine the factor structure, reliability, and validity of the Child-to-parent Violence (...) Questionnaire, parents’ version, in a sample of Spanish parents of adolescents. Moreover, the prevalence rates of the different types of violence and the reasons for violence were also examined. A total of 1,012 Spanish parents of adolescents aged between 12 and 17 years old were assessed using the CPV-Q-P. Data indicated a matrix of four factors with 14 items, assessing psychological violence, physical violence, financial violence, and control/domain over parents, and two factors with 8 items capturing the reasons for child-to-parent violence, with adequate psychometric properties. The more frequent type of violence was control and domain over parents, followed by psychological, financial, and physical violence, with no significant differences between mothers and fathers. Otherwise, instrumental reasons were more frequent than reactive types, with no differences between mothers and fathers. The CPV-Q-P is a useful instrument to assess child-to-parent violence from the parents’ perspective in both professional and research settings. (shrink)

This volume is published concurrently with the one reviewed below and together they unite a number of Quine's previously scattered papers into two compact volumes; this volume deals with his more philosophical work while the other is concerned with more purely technical logical studies. The twenty-one essays cover the period 1934-1964 and none have appeared between hard covers before. Several of the articles—"The ways of paradox," "Foundations of mathematics," "On the application of modern logic," and "Necessary truth"—are essentially (...) popular expositions. The others are generally more restricted in both scope and appeal, and deal with the ontology of the sentential calculus, truth by convention, implicit definition, modal logic, and ontological reduction in the sciences. Several articles concern the philosophy of science directly: "On simple theories of a complex world," "Posits and reality," and "The scope and language of science." This fine collection will be of significant help in presenting the work of a distinguished philosopher to a wider audience, as well as providing the professional with a source of discussion.—P. J. M. In the preface to the Tractatus Wittgenstein acknowledged ‘Frege's great works’ as one of the two primary stimulations for his thoughts. Throughout his life he admired Frege both as a great thinker and as a great stylist. This much is indisputable. What is disputable is how he viewed his own philosophical work in relation to Frege's and, equally, how we should view his work in this respect. Some followers of Frege are inclined to think that Wittgenstein's work builds on or (...) complements that of Frege. If that were true it would be plausible to suppose that the joint legacy of these two great philosophers can provide a coherent foundation for our own endeavours. But it is debatable whether their fundamental ideas can be synthesized thus. The philosophy of Wittgenstein, both early and late, is propounded to a very large extent in opposition to Frege's. They can no more be mixed than oil and water – or so I shall argue. (shrink)

Lynch O.P.In question 63 of the tertia pars, Thomas Aquinas defines the so-called character that is conferred by certain sacraments (namely baptism, confirmation, and holy orders), as a secondary effect caused by the sacraments, with grace itself identified as the primary effect. As separated instruments of the humanity of Christ, in his mature work in (...) the Summa theologiae Aquinas argues that all Christian sacraments are perfective instrumental causes of the effect of the Incarnation in the human persons who receive them.1 As instrumental means by which these effects are conferred, therefore, for Aquinas the sapiential intelligibility of the sacraments is found in the larger sweep of sacra doctrina and the Incarnation, wherein the ratio and necessity of these larger theological themes contextualize Aquinas's understanding of the necessity of the sacraments themselves as instrumental extensions of the same.2 Taken in itself, aspects of what would later become the question of sacramental character for Scholastics first emerged as a doctrinal topic in the context of patristic debates over the permanence of the sacrament of baptism.3 In the Latin West, the influence of Augustine's distinction between the permanence of the sacramentum received in baptism and its use in charity proved to be a ubiquitous influence [End Page 1337] for later Latin theology.4 Concerning the metaphysical nature of this character, however, in both his Sentences commentary and Summa theologiae, Aquinas joins an ongoing conversation among thirteenth-century Scholastics about the utility of the Aristotelian categories for describing sacramental character. What is sacramental character specifically, and how should this be understood in relation to the wider speculative complexities of grace, the infused virtues and gifts, and beatitude? Because of continued controversy over the role of Aristotelianism in theology during the second half of the thirteenth century, many Scholastics of Aquinas's generation analyzed the quiddity of sacramental character from an Aristotelian perspective, working to describe the doctrine inherited from the Church's tradition using Aristotelian terminology. As will be shown, although some argued that sacramental character should be understood as a habitus, Aquinas argues that character is better understood as a power.The second and third sections of this article will explore the development of Scholastic accounts of the quiddity of sacramental character during the medieval and early modern periods, and the wider theological implications of these conversations. In the final analysis, it is my aim to show not only that Aquinas's account of sacramental character is intrinsically fitting on theological grounds, but that Aquinas's account of sacramental character as a qualitative power conveys and preserves the received theological tradition more effectively than the options proposed by some of his contemporaries. With these larger objectives in mind, the first section of this article serves as a necessary historical prelude—what, precisely, is at stake from the wider perspective of the Church's theological tradition in these medieval and early modern Scholastic debates? Accordingly, the first section will provide an overview of the historical development of the doctrine of sacramental character, focusing specifically on the way in which certain Augustinian doctrinal concepts and linguistic usages developed in later medieval usage and ultimately came to be incorporated into the framework of Peter Lombard's Sentences—the text that would serve as the common backdrop for the Scholastic conversations about the "quiddity" of this same doctrine.In the following, therefore, this article will begin by examining the origins of the doctrine of sacramental character in Latin theology in Augustine's baptismal theology. Objectives: To assess French district nurses’ opinions towards euthanasia and to study factors associated with these opinions, with emphasis on attitudes towards terminal patients.Design and setting: An anonymous telephone survey carried out in 2005 among a national random sample of French district nurses.Participants: District nurses currently delivering home care who have at least 1 year of professional experience. Of 803 district nurses contacted, 602 agreed to participate .Main outcome measures: Opinion towards the legalisation of euthanasia , attitudes towards terminal patients (...) .Results: Overall, 65% of the 602 nurses favoured legalising euthanasia. Regarding associated factors, this proportion was higher among those who discuss end-of-life issues with terminal patients , who consider competent patients should always be told their prognosis and who value the role of advance directives and surrogates in end-of-life decision-making for incompetent patients . Women and older nurses were less likely to favour legalising euthanasia, as were those who believed in a god who masters their destiny.Conclusions: French nurses are more in favour of legalising euthanasia than French physicians; these two populations contrast greatly in the factors associated with this support. Further research is needed to investigate how and to what extent such attitudes may affect nursing practice and emotional well-being in the specific context of end-of-life home care. (shrink)

For all his peculiarities, man is not the only animal who knows. But he is, as far as it is possible to judge, the only animal who knows that he knows, and who seeks to understand the nature and meaning of the act of knowing itself. Michael Polanyi has been engaged in this search for more than twenty years, and the book here under review is an ‘interim report’ on the development of his thought since the publication of Personal (...) Knowledge and The Study of Man just ten years ago. Its three short chapters are substantially the Terry Lectures which he delivered at Yale University in 1962. This review will attempt no more than to summarise the central theme of Polanyi’s argument. A detailed, critical analysis of its philosophical significance is beyond the competence of a biologist. We are reviewing and summarizing evidence for the processes of acquisition of information outside of conscious awareness (processing information about covariations, nonconscious indirect and interactive inferences, self-perpetuation of procedural knowledge). A considerable amount of data indicates that as compared to consciously controlled cognition, the nonconscious information-acquisition processes are not only much faster but also structurally more sophisticated in the sense that they are capable of efficient processing of multidimensional and interactive relations between variables. Those mechanisms of nonconscious acquisition of (...) information provide a major channel for the development of procedural knowledge which is indispensable for such important aspects of cognitive functioning as encoding and interpretation of stimuli and the triggering emotional reactions. It has been proposed that inferring personal authorship for an event gives rise to intentional binding, a perceptual illusion in which one’s action and inferred effect seem closer in time than they otherwise would . Using a novel, naturalistic paradigm, we conducted two experiments to test this hypothesis and examine the relationship between binding and self-reported authorship. In both experiments, an important authorship indicator – consistency between one’s action and a subsequent event – was manipulated, and its effects on binding (...) and self-reported authorship were measured. Results showed that action-event consistency enhanced both binding and self-reported authorship, supporting the hypothesis that binding arises from an inference of authorship. At the same time, evidence for a dissociation emerged, with consistency having a more robust effect on self-reports than on binding. Taken together, these results suggest that binding and self-reports reveal different aspects of the sense of authorship. (shrink)
